Reuters Botches Iraq Story
The news agency reported "They've taken control" of Ramadi, until this was denied by US troops.
First they called the terrorist "rebels" and "militants":

Iraqi militants attacked a US base and a local government building with mortar rounds and rockets in Ramadi, west of Baghdad, on Thursday, before holding ground on several central streets, residents said. Around 400 heavily armed, masked men were patrolling the main thoroughfares of the city...
But it was obvious from their own reporting it was terrorists:
Leaflets were distributed and posted on walls declaring that al Qaeda in Iraq, the group led by Jordanian militant Abu Musab al-Zarqawi, was taking over the city..."Al Qaeda in Iraq is taking control of Ramadi," one of the leaflets read.
They later changed their story to a "Brief Assault" after US Military denied the bogus story:

Insurgents launched a brief assault west of Baghdad on Thursday, firing mortar rounds and rockets at a U.S. base and local government buildings, the day after Washington unveiled its new strategy for victory in Iraq....
The U.S. military, which together with Iraqi forces launched a new operation in the area round Ramadi on Wednesday, played down the assault.
"Reports of insurgents taking control of Ramadi are completely unsubstantiated," Marines Captain Patrick Kerr said in an emailed statement. "There have been a few sporadic small-arms engagements, but nothing out of the ordinary."
